How To Create A Form - Explained

Forms are primarily used on Skynamo to capture important customer data. The big benefits of forms are that they can be customised according to the needs of your company. This article explains how forms are created.

Note, forms can only be created from Skynamo Insights (i.e. the web portal).


Creating a form

Navigate to the Forms section:

 

And select Add Form:

 

Here you will be presented with the following page:


Please proceed with giving the form an appropriate Name which has bearing to the associated questions and answers required. I have named my form: "Coffee Machine Survey"


I have added a divider, purely for neatness and as a means to divide the form into separate parts (should your form require this).

 

Adding questions or fields to a form

Now we are ready to start asking the needed questions to reach the preset outcome based on the answers provided.


Default settings for question fields

You have multiple options (on the left-hand side under "Add a question") for question types, which varies from a basic test text field to a signature field. Before these fields are discussed, it is important to explain the settings available for most of the fields - these appear on the right-hand side of the screen, and look as follows:


Mobile settings – these settings influence end user ability on mobile

  • Visible on mobile – determines if users will be able to see the field on their mobile devices.
  • Editable on mobile – determines if users will be able to edit the question on their mobile devices.


Other settings -

  • This field must have a value – If this is ticked, the user will not be able to submit the form, unless this question is filled in.
  • Can search on this field – allows information to be searchable content.
  • Can filter on this field – all end users can find this information using the in-app filtering options.


Different question field types

The following example will be used to explain what each of the possible question options can be used for.

I have added a text field for my first question - Nr. of Nozzles on Machine.


The answer received here may have a value as a number or text  - which is why using a text field is really flexible data type.

 Next question is a Number field to capture the Retail value of the machine:


The answer expected here should be a monetary value, which may include cents, eg. R 24889.99 - which is why I set the number of decimals to two. 


Now we are ready to use a Dropdown on our form. Once you click on Dropdown, you will be presented with the following options:


Single Select definition -> Presents the user with multiple options to choose from which 1 option can be selected. This saves a lot of time and streamlines the process of completing a form.

Multi Select definition -> Presents the user with multiple options to choose from which more than 1  option can be selected.

Nested Single Select definition -> Presents the user with multiple options to choose from.
 1 option can be selected and upon selection, another level of options or data set will opens and a new option can be selected.

Nested Multi Select definition ->Presents the user with multiple options to choose from where more than 1 option can be selected and upon selection, another level of options or data set, opens up and new options can be selected.

 In this example, I am requesting the type of Beans used, with the options to set showing on the right side of the screen


 

Should your form have different divisions / sections, you may use Labels as a method of indication.

I am using a label in this example as an Employee Information Section


Labels can also be used for fine print in an agreement eg. orders, quotes when Customer Fields are being amended.

 Now we can link a date with this survey on which it was completed:


 

Lastly, I am adding a signature data field for a manager to sign off on the form that all was captured correctly:

 

This field also has an option to the right side of the screen, which states: "Finishes form" - which, in essence, means that when this field is completed, the form automatically closes and sends to our Servers and a copy of the form is sent to the rep (pending setup on Configuration page regarding form recipients).


This, as well as other fields, may be made compulsory to complete by enabling the option: "This field must have a value".

And click on the: "Create" button to make this form view-able to reps within roughly 5 minutes.

 


 (Note that it is good practice to save often when creating forms!)
