How to create a form - Web

1. On the Skynamo web interface, navigate to the Forms tab:

2. Click on Add form:

3. At the top, under New form, enter a name for the form:

4. Add your first question on the left-hand side:

5. After you have added your question, you can name it (either in the question block itself or on the right-hand side):

6. Configure your question field on the right-hand side:

Each question field has a slightly different set of configurations. You can see the full explanation for all of the different types of configurations here

7. Keep adding questions on the left and configuring them on the right until your form is complete

8. Click Create at the bottom of your screen:
